Two-Day Basic Musculoskeletal Ultrasound Workshop
April 16, 2012: 5:00 p.m. – 9:30 p.m.

The basic musculoskeletal ultrasound course is designed for those who have limited experience with musculoskeletal ultrasound. It is designed to introduce basic ultrasound principles and provide the participants the opportunity to learn how ultrasound could be used in their practice. This course will cover major body regions and review the basic protocols for examining the various joints of the body. Note: Space is limited to 40 participants and registration will be on a first come, first serve basis.

Advance Musculoskeletal Ultrasound Workshop
April 17, 2012: 1:30 p.m. - 5:45 p.m.

The advanced musculoskeletal ultrasound course is designed for those who already use musculoskeletal ultrasound in their practice. It is designed to briefly review basic ultrasound principles and provide an opportunity for participants to expand their knowledge and skills. Note: Registration will be on a first come, first serve basis. Space is limited to 40 participants and is available only to those who have already taken a MSK US beginner course or who has at least 6 months clinical experience with MSK US.

Workshop Fee: $250

Accommodations

Educational sessions and exhibits will be held at the Louisville Marriott Downtown Hotel in Louisville, Kentucky. The hotel is located next to the Fourth Street Live! entertainment district, featuring great restaurants, bars and live Jazz music. It is also within walking distance of many museums, recreational activities, numerous shops, historic attractions and only 8 miles from the Louisville International Airport (SDF). This lovely hotel offers a host of amenities, services, and new this year all AOASM guest rooms reserved under the discounted group rate offer daily complimentary high-speed Internet access during the conference dates.

Guest room amenities include a hairdryer, coffee maker/tea service, iron and ironing board, digital individual climate control, luxurious bedding, flat screen television with cable/satellite TV channels, including HBO, pay-per-view and movie/video rental, and refrigerator accommodations are available upon request. Dining amenities in the hotel include Starbucks Coffee House open for breakfast and lunch; the BLU Italian Grille open for breakfast, lunch, and dinner; and Champion’s Sports Bar & Grill open for lunch and dinner. Hotel Reservations

AOASM works diligently to provide attendees with the most convenient hotels at a good value. AOASM contracts with the hotel to hold sleeping rooms for use by our attendees. If AOASM does not meet the room block, penalty fees could be incurred. Please help AOASM keep costs down by making your reservations at the Louisville Marriott Downtown Hotel, 280 West Jefferson, Louisville, KY 40202; Phone: +1-502-627-5045 or Toll-Free: +1- 800-266-9432. When reserving your room via phone, please refer to the AOASM Clinical Conference in order to be eligible for the discounted rate. We encourage all attendees at the Clinical Conference to book their reservation at the headquarters hotel.

To reserve your room online, please go to the AOASM annual conference website and click on the Clinical Conference direct reservation page labeled: American Osteopathic Academy of Sports Medicine Hotel Reservations. In order to confirm a room assignment for attendees, the hotel requires a (first or last) night’s pre-payment deposit when reserving your room. The hotel reservations deadline is March 15, 2012. Room rates cannot be guaranteed after that date. The AOASM discounted room rate is $159 for single and double occupancy; $169 for triple occupancy; and $179 for Quadruple occupancy. The room rate is exclusive of occupancy tax, which is 15%. Quoted rates will be offered, based on availability, to all attendees 3 days prior and 3 days after the conference dates (4/15/12 to 4/24/12). RESERVE YOUR ROOM EARLY in case the room block at the hotel sells out prior to March 15, 2012. Rooms will be reserved on a first come, first serve basis. Check-in time is 4:00 p.m. and checkout time is 11:00 a.m.

Hotel Cancellation policy: Guest must notify Hotel prior to 6:00 p.m. on the day of arrival, to not forfeit the one-night deposit.

Ground Transportation

The following taxicab and car rental options are permitted to provide ground transportation to Louisville International Airport travelers and customers. Educational Credits

AOA Credits
AOASM has requested that the AOA Council on Continuing Medical Education approve the pre-conference workshops for up to 12.5 hours of AOA Category 1-A CME credits. The Clinical Conference program has been requested for up to 27.0 hours of AOA Category 1-A CME credits. (Approval is currently pending.)

BOC Credits
The American Osteopathic Academy of Sports Medicine is recognized by the Board of Certification, Inc. to offer continuing education for certified athletic trainers. This program has been approved for up to 24.0 hours of continuing education credits.

Physical Therapy Credits
Continuing education (CE) requirements for physical therapists vary from state to state. Most states that mandate CE for renewal of licensure have a process in place where an individual PT/PTA licensee may petition directly to the state licensing agency for acceptance of a continuing education course. Check with your state licensing board for the proper procedures and forms.

Examinations

Sports Medicine Examination for Certification of Added Qualifications (CAQ) AOASM will be offering the Sports Medicine Examination for Certification of Added Qualifications (CAQ) from 1:00 p.m. to 5:00 p.m. on Tuesday, April 17, 2012. The Recertification Exam will also be offered from 1:00 p.m. to 3:00 p.m. You must pre-register with the American Osteopathic Association (AOA) to sit for either exam. AOASM does not coordinate the registration for the exam. The goal is to have all Sports Medicine Fellowship Programs participating with a poster presentation, a case podium presentation, or a research podium presentation.
