This week's Quick Hit topic for our softball/baseball theme is a radiology case. Please review the question stem and image below and leave your answer in the comments!

A 26 y/o minor league pitcher presents to the clinic with 2 days of medial elbow pain starting after he threw a curveball and heard a pop. There was immediate onset of pain and swelling. He has a positive valgus stress test at the elbow. An MRI is ordered (see image below).

What is the diagnosis? How do you counsel the patient about management?

Answer: UCL rupture; the recommended management in a high level pitcher is UCL reconstruction (aka Tommy John surgery)
